The Unified Health System (SUS) created an identification card for Brazilian citizens and residents of the country. The idea of the SUS card is to organize patients so that they can use the public health system.
This card contains all the dates and places where the patient received care, which services were provided, by which professional and which procedures were performed.
The idea of the card is to organize and systematize the service provided, since it generates a national identification number that facilitates access to the service network. Thus, the card facilitates the communication of health services.
With the SUS card, the user will be able to access the Citizen Health Portal and thus consult their history of records of health actions and services in the system.
Information is included, such as: hospital admissions, with data on outpatient care of medium and high complexity and purchase of medicines in the Popular Pharmacy program. Names of the health professionals who attended you, the period, the name of the hospital and the clinical and surgical procedures performed.
The card is free and can be requested in person at the Municipal Health Department, or at a SUS Health Unit or over the internet.
For the issuance of the card, it is necessary to present:
Identity document (RG);
CPF (Cadastro de Pessoa Física);
Birth certificate or marriage certificate.
In some municipalities, proof of residence is required, such as a water, electricity or telephone bill
How to apply for the SUS card over the internet?
The request is simple and the card will be ready for printing immediately.
Access the official website of meuDigiSUS at https://meudigisus.saude.gov.br/menu/home;
Click on "Login" next to "Hello - Login to view your health data";
Log in with your “gov.br” Portal registration (www.gov.br). If you do not have this registration, create a new account;
Check the information that “meuDigiSUS” can access and, being in agreement, click on “Authorize” to proceed;
On the main page of "myDigiSUS", click on "My health";
Next to your name and “My National Health Card”, click on “Access”;
With the card displayed in the browser, click on the icon in the upper right corner of the screen to download it. The file must come in the “png” format;
On the same page as the previous step, you can view your card's QR code;
And ready, just make the impression.
Another option is to download the Connect SUS app, with the login the system downloads your SUS card and makes it available for online presentation.
